		RULES OF ENROLLMENT For the enrollment you should send to the ISU Enrollment Committee the full set of documents according to the list. You can do it personally or by the registered postal packet to the address: 01135, Ukraine, Kiev, Sholudenko street 1b, ISU Enrollment Committee. In the application mark your exact postal and electronic address for further correspondence, indicate the form of study (full-time or part-time) and the speciality, which you have chosen.	List of necessary documents: statement on the name a Rector, questionnaire, agreement (in 2 copies), originals of certificate from school or diploma about the education got before secondary or higher education, medical certificate (form 086-у), 6 photos 4х4 sm, official document which gives a right to the privileges (at presence of), notarized notarial copy of passport, certificate with an identification code (for the citizens of Ukraine), military ticket (for the citizens of Ukraine - men). Procedure of receipt is regulated by rules.
